Beyond Referral: Practical Psychological Strategies for Primary Headache

This presentation examines psychological approaches to primary headache management within a biopsychosocial framework. Psychological interventions are conceptualised according to three potential therapeutic targets: distress generation, distress reaction, and central nervous system dysregulation. Approaches discussed include cognitive-behavioural therapy, psychoeducation, acceptance and commitment therapy, relaxation training, biofeedback, mindfulness, and clinical hypnosis. Evidence from systematic reviews and meta-analyses suggests that psychological interventions can improve headache-related outcomes, although effects vary across interventions, age groups, and headache diagnoses. The presentation highlights the need to move beyond a narrowly biomedical or traditional CBT-focused approach toward mechanism-informed, developmentally appropriate psychological interventions integrated with headache care.
